Alzheimer’s Community Care Organization provides no-cost and low-cost dementia-specific, integrated services and resources for patients with Alzheimer’s disease or a related neurocognitive disorder, and their caregivers. The Organization is community-based and non-profit, IRS 501(c)(3) providing services designed to meet health, social, and therapeutic recreational needs. Our services help patients and caregivers maintain quality of life in the community. The Organization provides caregivers with tools and education to care for their loved ones at home, delaying or eliminating costly nursing home placement. Services include Specialized Alzheimer’s Day Care and Service Centers, Family Navigators, ID Locator Services for patients at risk to wander, dementia-specific education and training for family caregivers, health and allied professionals and health professions students, law enforcement, etc., Case Management, Patient and Caregiver Advocacy, and Annual Education Conference. Services: Family Navigators at no cost to family, assisting caregivers with education and linkage to needed services, 24/7 Crisis Line (for non-medical, dementia-related concerns), Education and Training, Caregiver Support Groups. People Served: Patients with Alzheimer’s disease or a related neurocognitive disorder, and their caregivers.
